Staffed by registered nurses and nutritionists, the Diabetes and Nutrition Center offers an education program designed to empower diabetics, in partnership with their physician, to manage their diabetes throughout their lifetime. In order to meet the individual needs of each patient, the Center offers group as well as one-on-one instruction. Outpatient nutritional counseling is available for a variety of medical conditions such as renal disease or disturbances of cholesterol or triglycerides. Also available is weight management counseling. Housed inside the Diabetes & Nutrition Center, St. Francis offers an outpatient American Diabetes Association recognized Diabetes Self Management Training program (DSMT) designed to assist people with diabetes and their caregivers in our service area.
A physician referral is required. The Center is open 8:30 am to 5 pm Monday through Friday by appointment. Other times are available for education on an individual basis. Costs for program participation varies based on patients individual insurance provider.
