MLK Health Center and Pharmacy offers free diabetes prevention and self-management programs to community members.
The Diabetes Prevention Program is a year-long program with 16 weekly core sessions and 6 monthly follow-up sessions with trained lifestyle coaches. Participants are empowered to take charge of their healthy by making achievable and realistic lifestyle changes to reduce the risk of developing type 2 diabetes. New classes begin each January.
“Living Well with Diabetes” is a diabetes self-management education class that teaches participants how to enjoy life while managing their diabetes.* Four weekly sessions cover the following:
- Understanding diabetes, the use of meters, monitoring and medications
- Creating a healthy meal plan
- Being active
- Maintaining overall health including complication prevention, problem solving, stress reduction, and getting the emotional support needed
* Diabetes self-management education and support (DSMES) is a critical element of care for people with diabetes. DSMES is the ongoing process of facilitating the knowledge, skills, and ability necessary for diabetes self-care, as well as activities that assist a person in implementing and sustaining the behaviors needed to manage his or her condition on an ongoing basis, beyond or outside of formal self-management training.
