Diabetes education is executed in a series of appointments and a 3-day group training. Diabetes care includes several services that are available upon request. Please check the website for further details about each service. One-on-one sessions are taught by an RN, Diabetes Educator, and/or Registered Dietitian. Group training sessions are taught by a multidisciplinary team of Certified Diabetes Educators, Registered Nurses, Registered Dietitians, Exercise Physiologists, and Health & Lifestyle coaches.
Vantage members can participate in the program at no cost, although if patients have other health care providers there may be a cost involved. Patients get access to annual diabetic foot exam including screening for eligibility of diabetic shoes and inserts; healthy cooking demonstrations in our kitchen including Gluten-Free Lunch & Learn Series, recipe substitution ideas, and meal planning; participate in grocery store tours with a Registered Dietitian; fitness assessment and an exercise prescription with an Exercise Physiologist / Health & Lifestyle Coach; includes a follow-up to check on progress of your goals; group exercise classes and access to gym equipment.
