Community-based blood pressure monitoring screening site providing onsite blood pressure screenings, health education resources and referrals to clinical support to ensure follow-up care. This program aims to increase the number of community-clinical linkages within communities and throughout our state in order to improve chronic disease prevention, care and management for individuals with uncontrolled hypertension and not under the care of a medical provider.
