Terrebonne General Medical Center (TGMC) offers a comprehensive diabetes self-management program to serve the needs of the diabetes population in our region. Our team of educators understands that diabetes self-management is a life-long journey. Our educators will work closely with you to provide the knowledge and skills necessary to anchor you on course towards your destination of success in diabetes control. The program’s overall objective for diabetes self-management education is to empower you in decision-making, self-care behaviors, problem solving and collaboration with the health care team in order to improve clinical outcomes, health status, and quality of life. Based on the needs of the patient and family, and the recommendations of the referring physician, we will provide you with a complete educational/training experience.
A doctor’s order is necessary. Call the Diabetes Management Center at 985-850-6200 and we will be happy to take the necessary information to obtain orders from your physician. Costs for program participation varies based on patients individual insurance provider. Office Hours: Monday – Friday, 7:30 am – 4 pm.
