Diabetes ECHO Application

I understand I must agree to the following:

• Participant must submit and present at least one patient case during the calendar year
• Participant will complete two surveys (pre and post survey) to assess efficacy of the program
• Participants who wish to receive CEUs must also complete a brief post-session survey for each session attended

In order to experience Diabetes ECHO at its maximum capacity, we are requiring all registrants to present a patient case presentation. ECHO staff will reach out to registrants to assign a case presentation date.

Case presentation is a key component of the Project ECHO model and critically important for knowledge building. It offers an opportunity to brainstorm evidence-based solutions or share successes with colleagues and other professionals within the network. During an ECHO session, registrants present real (de-identified) patient cases to specialists and other participants for expert advice, discussion, and recommendations. As a result, everyone learns as knowledge is tested and refined through a local lens.

In order to join our sessions, all registrants must create an account on the new iECHO platform. You will receive an email from iECHO, there you will be able to sign in and access the Diabetes ECHO program.

We encourage all participants to explore the exciting features on our iECHO platform! iECHO is the web-based, proprietary teleECHO program management software and database developed and managed by the ECHO Institute. The system’s purpose is to track the ECHO movement’s programmatic activities around the world. Due to its key role in tracking the movement, iECHO usage is mandatory for partners conducting Project ECHO activities.

Project ECHO® collects registration, participation, questions/answers, chat comments, and poll responses for some ECHO programs. Your individual data will be kept confidential. These data may be used for reports, maps, communications, surveys, quality assurance, evaluation, research, and to inform new initiatives.
