Diabetes ECHO Application

Participant expectations are as follows:

• Participant will attempt to attend all sessions.
• Participant will complete pre- and post-surveys to assess efficacy of the program.
• Participants who wish to receive CEUs must also complete a brief post-session survey for each session attended

In order to experience Diabetes ECHO at its maximum capacity, we are requiring all patient-facing registrants to present a patient case presentation and select a session date when they would prefer to do so. Case presentation details will be shared after registration.

All registrants must create an iECHO account.

iECHO is the web-based, proprietary teleECHO program management software and database developed and managed by the ECHO Institute. The system’s purpose is to track the ECHO movement’s programmatic activities around the world. Due to its key role in tracking the movement, iECHO usage is mandatory for Project ECHO activities. Individual data will be kept confidential.

Following registration, you will receive a “Welcome” email from Well-Ahead Louisiana with information on creating an iECHO account. After account creation, you will be able to sign in, access the Diabetes ECHO program, and join future sessions.
