Improving Asthma Outcomes for Louisiana Residents

The Asthma Control Program aims to supports individuals with asthma by offering self-management education, environmental assessments, medication support, and coordination with health care providers to reduce asthma-related hospital visits and improve quality of life and outcomes for asthma patients and their families.

  • Services may include:
  • Asthma education and management support
  • Home or virtual environmental assessments
  • Medication adherence monitoring
  • Referral to social or clinical services as needed

This program serves eligible residents of Louisiana who have been diagnosed with asthma. Participation is voluntary and consists of no-cost services to the participant.

  • Who’s eligible to enroll into the Asthma Program?

    Program eligibility is any adults and children that have been diagnosed with Asthma.

  • How does the program target the community?

    The program typically targets being vendors/exhibitors at family-oriented or school-aged children type events in hopes of increasing possible program enrollments. The program also considers events with healthcare professionals and schools to gain provider or educator referrals for participation.

  • How does enrollment work?

    In addition to the ways to enroll below, there is also a self-enrollment option through the CareHalo app platform. Additionally, if the patient is already enrolled in one of the other Well-Ahead programs, Diabetes, Heart Disease, WISEWOMAN, the participants can elect the option in the CareHalo app to be considered for the Asthma Control Program. From there, our Asthma Control Rep or Nurse Consultant contacts them for further enrollment steps.

There are several ways to enroll:

  • Provider Referral
    1. Complete both the Asthma Control Consent Form and Asthma Control Referral Form.
    2. Obtain the patient/legal guardian signature and contact information.
    3. Fax completed forms to 1-800-621-1871.
  • Patient Self-Enrollment
    1. Providers provide the Asthma Control One-Pager with QR code to the patient/legal guardian.
    2. The patient/legal guardian will scan the provider-specific QR code to complete self-enrollment.

    This option should be reserved for reliable patients who are able to enroll independently.
