Through the DSMES via Telehealth Project, Well-Ahead Louisiana is partnering with health care systems throughout the state to expand the use of telehealth technology to deliver DSMES programs in underserved areas. Rural health clinics and federally qualified health centers interested in partnering with Well-Ahead Louisiana and a Diabetes Self-Management Education and Support (DSMES) provider to establish a DSMES via Telehealth connection at their facility should fill out the application below to be considered for this Project.
