The American Heart Association recently praised Well-Ahead Louisiana’s Population Health Cohort for submitting heart disease data for their Target: BP™ project. Nine of the 13 Rural Health Clinics participating in the Cohort registered and participated in Target: BP, which was encouraged but not required as part of their participation.
Target: BP™ is a national initiative formed by the American Heart Association (AHA) and the American Medical Association (AMA) in response to the high prevalence of uncontrolled blood pressure. Target: BP helps health care organizations and care teams, at no cost, improve blood pressure control rates through an evidence-based quality improvement program and recognizes organizations committed to improving blood pressure control.
