Online Program for Diabetes Prevention

The National Diabetes Prevention Program (National DPP) is an evidence-based CDC-recognized lifestyle change program offered in organizations across the nation that can cut a participant’s risk for developing diabetes in half. However, some of our residents face health disparities that make accessing these programs more difficult. In order to address these challenges, Well-Ahead Louisiana provides organizations a virtual platform to offer the curriculum to those in need.

Now’s the Time National DPP is an online platform that utilizes the CDC-approved curriculum to teach participants how to implement lasting, healthy lifestyle changes to prevent type 2 diabetes. It removes the barrier of transportation and scheduling conflicts for participants, allowing them to complete sessions based on their schedule from the comfort of their own home. Through the app or website, participants can access an engaging video curriculum, track their meals and exercise, and communicate with their lifestyle coach and other cohort participants.

By offering a virtual solution, organizations can improve enrollment and retention in their National DPP by allowing lifestyle coaches to reach a higher number of participants than they could reach through an in-person program. Programs that utilize Now’s the Time National DPP get access to a private, secure mobile app and website experience that includes:

  • 26 video lessons via mobile phone or computer over 12 months (weekly lessons for the first four months followed by monthly lessons)
  • Easy to use dashboards and data collection tools for program coordinators and lifestyle coaches.
  • An online community of peers for participants that provides encouragement and creates accountability through shared goals and objectives
  • Meal photo-journaling, physical activity and weight loss tracking
  • Customized feedback and support for participants from the Lifestyle Coach on progress

Get Started

For an organization to be eligible to deliver the National DPP through the online platform, there are only two requirements: CDC recognition and a lifestyle coach to lead the program. Well-Ahead can help your organization with both! Connect with us to get started.

  1. First, Well-Ahead can assist your organization with the application process to be recognized by the CDC as an online provider. The Diabetes Prevention Recognition Program standards explain how an organization may apply for, earn and maintain CDC recognition. For organizations currently offering the National DPP, you can update your application to be able to utilize the online platform for delivery.

  2. Then, Well-Ahead can help you identify a wellness champion in your organization and work with you to get them trained as a lifestyle coach and even provide funding, as needed.

  3. Once your organization has met the requirements, Well-Ahead will provide access to the platform for your program coordinators and coaches. We will train your team on utilizing the Now’s the Time National DPP platform and provide best practices for online delivery.


Spread the Word

To have a successful program, you’ll need to recruit participants to join your program, and Well-Ahead can help you spread the word!

Use our Now’s the Time communications tools to help educate your community on prediabetes. These tools can also be shared with organizations in your community to help spread the word. On nowsthetimela.com, residents can take the quiz in less than a minute to learn about their risk for prediabetes. Those that are at risk, are then encouraged to fill out a National DPP referral form. When your program is open for enrollment, your organization will be added to the form to receive referrals.

Participating in a Virtual National DPP

To be eligible for the Now’s the Time National DPP, participants must meet the following criteria:

  1. Must be 18 years or older
  2. Must have a body mass index (BMI) of 25 or higher (23 or higher if Asian American)
  3. Cannot have been diagnosed with type 1 or type 2 diabetes
  4. Cannot be pregnant
  5. Meets one of the following criteria:
    1. A blood test result within the past year indicating one of the following:
      1. Hemoglobin A1C: 5.7–6.4%
      1. Fasting plasma glucose: 110–125 mg/dL
      1. 2-hour plasma glucose (after a 75 g glucose load): 140–199 mg/dL
    1. Diagnosed with gestational diabetes during a previous pregnancy
    1. Received an at-risk result on the Prediabetes Risk Test
