The topics include Diabetes Basics, Diabetes Concept, and Diabetes Life. A graduation ceremony is held after completion of the 3 Part Series. Their family nurse practitioners specialize in diabetes management and is available to assist in improving blood glucose control during hospitalization. While hospitalized, one-to-one educational sessions are available from our certified diabetes educators. The high-risk diabetic population includes people who are newly diagnosed, have uncontrolled diabetes, have gestational diabetes, recently started taking insulin. Savoy Medical Center (SMC) New Horizons offers specialized treatment for those suffering from emotional and behavioral disturbances. Their experienced team of professionals, including our Psychiatrists, a Certified Psychiatric Nurse Practitioner, RNs, LPNs, a Recreational Therapist, Licensed Social Workers and Counselors, and Mental Health Technicians, will assist you in identifying, treating and implementing an individualized plan using a multidisciplinary approach.

They offer two levels of treatment for Adult and Geriatric patients, including Inpatient Hospitalization and an Intensive Outpatient Program.  Referrals can be made by Primary Care Physicians, Psychiatrists, Social Workers, Nursing Facilities and concerned family members. Following a referral, a free assessment will be made by our professional health care team to determine the most appropriate level of care for you or your family member.

The Family Tree Counseling Program is comprised of licensed counselors and counselors working towards licensure, who provide assistance and guidance to those who need assistance resolving personal, social, or psychological problems and difficulties.  These counselors are able to provide a variety of assistance to individuals, families, step-families, single parents, couples, senior citizens, teens and children ages three and older. Topics that counselors can assist with topics such as relationships, parenting challenges, LGBTQIA, coping with divorce, recovering from a crisis or disaster, coping with grief or loss, anxiety, depression, addictions, stress and anger management.

Staffed by registered nurses and nutritionists, the Diabetes and Nutrition Center offers an education program designed to empower diabetics, in partnership with their physician, to manage their diabetes throughout their lifetime. In order to meet the individual needs of each patient, the Center offers group as well as one-on-one instruction. Outpatient nutritional counseling is available for a variety of medical conditions such as renal disease or disturbances of cholesterol or triglycerides. Also available is weight management counseling. Housed inside the Diabetes & Nutrition Center, St. Francis offers an outpatient American Diabetes Association recognized Diabetes Self Management Training program (DSMT) designed to assist people with diabetes and their caregivers in our service area.

A physician referral is required. The Center is open 8:30 am to 5 pm Monday through Friday by appointment. Other times are available for education on an individual basis. Costs for program participation varies based on patients individual insurance provider.

West Jefferson Medical Center’s National Diabetes Prevention Program groups meet once a week for 16 weeks, then once a month for 6 months to maintain healthy lifestyle changes. During each session, your lifestyle coach will teach a lesson and lead a group discussion. For example, you will learn to eat healthy, add physical activity to your life, manage stress and stay on track when eating out. 

West Jefferson Medical Center’s is part of the National Diabetes Prevention Program, led by the Centers for Disease Control and Prevention (CDC). It is proven to help people with prediabetes prevent or delay development of type 2 diabetes. As part of a West Jefferson Medical Center’s National Diabetes Prevention Program group, you will work with other participants and a trained lifestyle coach to learn the skills you need to make lasting changes. These changes include losing a modest amount of weight, being more physically active, and managing stress. Being part of a group provides support from other people who are facing similar challenges and trying to make the same changes you are. Together you can celebrate successes and find ways to overcome obstacles.

Some insurance plans will cover the cost for West Jefferson Medical Center’s National Diabetes Prevention Program. Check with your insurance provider to see if West Jefferson Medical Center’s National Diabetes Prevention Program is covered.

Our Lady of the Lake's Diabetes & Nutrition Center is a leader in the treatment of adult and pediatric diabetes and nutrition disorders. The center provides comprehensive programs including diabetes self-management training and medical nutrition therapy. Diabetes Self-Management Training Program (DSMT) is provided to clients with all types of Diabetes (Type 1, Type 2, secondary and gestational) and our team of certified diabetes educators, registered nurses and registered dietitians provide education and ongoing support so you can have a better understanding of how to live with diabetes. With a referral from your physician, you'll receive an initial assessment, knowledge/skill evaluation, an educational plan with measurable learning and behavioral objectives and insulin pump training (if applicable). Medical Nutrition Therapy (MNT) has registered dietitians that are available to provide individual medical nutrition therapy to people with nutrition-related problems, such as diabetes, high blood pressure, high cholesterol, kidney disease and weight management (including bariatric clients). This outpatient service provides an opportunity to outline a nutrition program that encourages long-term lifestyle changes to meet health goals. There are several physical and emotional issues to consider when living with diabetes. The Diabetes & Nutrition Center addresses all these issues in our free monthly Diabetes Education and Awareness Support Group. Those with diabetes and their families are invited to attend free sessions led by the diabetes educators. Topics range from the latest in diabetes equipment to healthy recipes. The diabetes support group meets on the second Tuesday of each month in the Our Lady of the Lake Auditorium from 6 p.m. to 7 p.m.

Before enrollment is completed, the primary care physician will need to be notified and approve the enrollment. In most cases, insurance will assist with the cost of this program (co-pays and deductibles apply).

The Diabetes Management Program at Lane Regional Medical Center is here for just that purpose and has all the answers you need to live a happy, healthy lifestyle with diabetes. Whether you have Type 1 or Type 2 diabetes, glucose intolerance or gestational diabetes, this program is tailored to meet your specific needs. Group education is combined with one-on-one attention, and you will learn management skills in accordance with the National American Diabetes Association Standards, such as home blood glucose monitoring, insulin administration, proper glucagon utilization, appropriate food choices and meal planning. Our three-step program is designed to assess your needs and the specifics of your condition, the introduce you to group settings with others in similar circumstances. Our team of professionals will help you stay on track with consultations available at your request.

The total cost for 10 hours of education and follow-up for 2 years = $812. Nine hours of group classes = $684. 

Physician's office must complete the required Physician’s Order Form provided to our office. Fax order to LRMC Diabetes Program at (225) 658-4486. Insurance benefits will be obtained by LRMC and patient contacted for an appointment. Most insurance companies, including Medicare, assist with the cost of diabetes education. Diagnoses of pre-diabetes and insulin resistance are not always covered. Medicaid does not pay for education. Medicaid and self-pay patients can be seen, but they must make payment arrangements with the business office.

The Touro Diabetes Center provides a comprehensive program of educational services and support to individuals and groups. The Diabetes Center works closely with physicians to identify each patient’s specific needs and helps them develop the self-management skills needed to control their disease. 

The educators in the Touro Diabetes Center are certified through the National Certification Board of Diabetes Educators, and the American Diabetes Association recognizes this program as meeting the national standards for diabetes self-management education. Our team also includes a licensed, registered dietitian to help coach patients through meal planning and weight control. 

To receive outpatient diabetes education, a physician’s order needs to be completed, signed and faxed to the Diabetes Center at (504) 897-7121. The Diabetes Center staff will contact the patient to schedule their first appointment. Medicare and most other insurance plans cover diabetes education and support services when medically necessary. Co-pays and deductibles may apply. The Diabetes Center also accepts private pay patients.

Overton Brooks VA Medical Center offers a Diabetese Self-Management Education (DSME) Program that offers help to Veterans. The program offers education regarding disease control and self-management. Medical Professionals will guide patients through the program using face-to-face counseling, telephonic assessments, goal setting and problem solving. These professionals will also use plain language to discuss complex disease processes and medical interventions. The DSME Program meets every first, third, fourth, and fifth Wednesday of each month between 8:00am-10:00am on the 10th floor of the medical center in room 10. Veterans who are interested in the program should consult their primary care provider for eligibilty and enrollment. All participants are asked to report to this appointment with their Accu-Check Aviva glucometer and a snack breakfast.

Based on effective efforts researched by the National Institutes of Health, the YMCA's Diabetes Prevention Program will help you learn about and adopt the healthy eating and physical activity habits that have been proven to reduce the risk of developing type 2 diabetes. Through the program you will receive support and encouragement from both a trained lifestyle coach and fellow classmates as you develop a plan for improving and maintain your overall well-being.

The 12-month group-based program consists of 16 one-hour, weekly sessions, followed by monthly sessions led by a trained lifestyle coach who facilitates a small group of people with similar goals. Topics of discussion are in areas such as healthy eating, increasing physical activity, reducing stress, problem solving, and much more. 

Based on research funded by the National Institutes of Health, the program has been shown to reduce the number of new cases of type 2 diabetes by as much as 58%. The reduction was even greater, 71%, among adults aged 60 years or older.

Check with your insurance provider as it may be a covered benefit. The cost of the program is $429.00. The YMCA does offer program scholarships for participants.

The Health & Wellness Center's Diabetes Education Program provides education and training services to patients with diabetes and those at high risk of developing type 2 diabetes. Accredited by the American Association of Diabetes Educators the program helps individuals improve their health outcomes as they learn to develop and maintain the skills of managing their illness. While working with our registered dietitian and registered nurse, participants will be assessed for any needs or concerns they may have about diabetes; develop the skills needed to manage their diabetes; receive an individualized education plan that will direct the appropriate diabetes self-management interventions and establish support strategies as it relates to their diabetes. 

Interactive group education sessions are also available through the Diabetes Education Program and focus on diabetes disease process and treatment options; incorporating nutritional management into lifestyle; incorporating physical activity into lifestyle; monitoring blood glucose levels and interpreting and using results; reviewing therapeutic treatments for effective diabetes management; preventing, detecting and treating acute and chronic complications; developing personal strategies to promote and maintain health and behavior change; pre-diabetes and insulin resistance; gestational diabetes and insulin training. 

A physician referral is required for admission to the program. Most insurance plans cover and pay for diabetes education and nutrition therapy if the patient meets the necessary requirements.

The Louisiana Heart Hospital (LHH) offers a free program to Louisiana residents who have been smoking since September 1, 1988 or before. Quitting smoking isn’t easy and it takes commitment and effective outside support.  Research indicates that individuals who participate in group cessation counseling have significantly higher quit rates, and LHH offers classes to participants that meet the above criteria.

Lung cancer causes more deaths in the United States than any other type of cancer.  Often time early lung cancer goes unnoticed as symptoms are difficult to identify. Lung cancer screenings for high risk patients increase the likelihood that this form of cancer is identified at an earlier stage. 

Who should be screened for Lung Cancer?  Individuals between the ages of 55-77 who are either a current smoker or who have quit within the last 15 years are good candidates for this screening.  The Slidell Memorial Hospital Imaging Center offers lung cancer screening for cash price at $95.00.  If you do not have insurance or have a high deductible, take advantage of this price.
