The Network serves as a conduit between healthcare professionals and continuing education, training, technical assistance and more—enabling you to better your practice and lead your patients into a healthier future.

By developing partnerships and connecting communities, you can help improve patients access to preventive and chronic care services, and reduce preventive diseases in our community.

Healthcare Workforce Recruitment and Retention
Throughout the U.S., there are geographic areas, populations and facilities with too few primary care, dental health and mental health providers and services. To help ensure our community members have access to healthcare services, we aim to connect our healthcare providers to workforce opportunities throughout the state of Louisiana.
Population Health Cohort
The Population Health Cohort is an exclusive collaborative quality improvement opportunity which supports the implementation of strategies aimed at improving population health within a primary care setting, with a specific focus on chronic disease related outcomes. It gives Louisiana providers and their facilities the opportunity to have hands-on assistance in implementing evidence-based practices that can improve their quality of care and their patients’ health outcomes.

Join the Provider Education Network
The Provider Education Network provides you access to community programs that you can refer and share with your patients to help prevent or manage their chronic diseases. Additionally, we connect providers to resources such as quality improvement strategies, patient education tools and workforce development opportunities. Furthermore, the Network can help you reach previously inaccessible patients by providing assistance using the growing trend of telehealth to expand medical care access to the rural areas that pocket our state.
Network members receive biweekly newsletters full of education and resources, as well as hot topics and upcoming events and deadlines relevant to Louisiana healthcare professionals. Members also are added to our Provider Education Network Alert List and receive timely Louisiana healthcare policy guidance and updates directly to their inbox.
